1st rotation: Gen Med Red
Yesterday, my registrar was running around the hospital looking for a syringe driver. The patient: a 80+ year old lady, BIBA post fall and has since been unconcious. CTB showed a gradually expanding SDH associated with hydrocephalus and the family decided not to go for surgery. Since pall care services were put in place, the nurses were giving the patient 4-5 breakthrough doses of morphine each night, thus why my registrar was looking for a syringe driver.
We had called for a pall care consult and the pall care nurse had come and told us that she only had 3 syringe drivers in the whole of the hospital, all currently being used. If we wanted to give our patient background morphine, we would have to set it up as an infusion pump.
In the afternoon, a patient under the white team died. My registrar's first question to the team: "Was your patient using a syringe driver?" It seemed that fate had deemed fit to give us a syringe driver at last but then when we came upstairs to give our intern the news...the patient had passed away. My intern was just certifying as we walked in the room.
Fate, playing at her little game?
Mrs. P,S passed away on 25/01/11 at 4:20pm. May she rest in peace.
