Today I attended a MET call for one of the other teams' patients. This gentleman was in the middle of physiotherapy when he collapsed on the floor with witnessed headstrike but no LOC. On initial assessment he was slightly altered with slight disorientation, maintaining his airway, good sats, tachycardic at 130 and hypertensive at 200 systolic. His background is right basal ganglia haemorrhage resulting in left hemiparesis and dysarthria.

ECG showed sinus tachy with no obvious cardiac ischaemia. Here's the fun bit. Put in an 18G cannula for him and took some blood then I ask the nurse to hang a bag of saline for him. One: there's no IV chart on the crash trolley which there should be given rehab patients never have existing fluid orders. Two: a person at the MET call who shall remain unidentified and unclassified asked me "Why are we giving fluids to this guy?"

Excuse me, I know we are in rehab but surely giving fluids to a collapse patient with tachycardia is standard treatment even if he is not hypotensive. No? What did everyone think? Fluids whilst we wait for ambos? Or sit on the tachycardic patient?
